Parenting is overwhelming enough. Then we have to keep track of all the medical information and appointments.
Oh, and let’s not forget about school/daycare info, especially if you have multiple kids in different schools.
I know it’s not just me.
Many parents I know have a lot of doctor’s visits to keep track of for their kids. And school information that also needs to be organized.
Here’s a free doctors visit printable tracker that you can use to keep track of your child’s medical information and prevent forgetfulness when it comes to essential appointments.

- Create a central place for all of your children’s doctor’s visits, including the date and time
- Keep track of any medications they are taking
- Make sure you have emergency contact information on hand in case anything happens
- Print out important dates (like immunizations) to keep with them at all times
- Write down any questions that come up during the appointment so you can ask the doctor later
Why A Doctor Visit Printable Tracker

As a mom with three kids, I wanted to find an easy and organized way to track their appointments.
The first thing I set out to do was find a Home Organizing printable on Pinterest, but there was nothing that I found that fit my needs. I wanted something colorful with pictures and clear sections for multiple doctors.
After looking at several apps, I realized that most Parents’ Planner only offers the option for one child and were not customizable (they didn’t have my kids’ names, gender, date of birth, etc.). That was a deal-breaker for me.
I then had the idea to create a customized planner that would track everything from Doctor Appointments & Medical Checkups to General Practitioners & Dentists. A planner where I could simply mark the time and location of their Doctor Visits, also keep note of any Doctor’s Note or Prescription required.

Keep Track of Medications

My kids have severe allergies, so having someone watch my kids is difficult. I need to make sure that the people who look after my children are fully informed about their allergies and what to do if something happens.
I started by filling out the girl-friendly version of the form with my Lil girl’s information and the boy version with my son’s.
I wrote their names at the top of each sheet. I included their food allergies below their name. He had one, and she had 5.
My kid outgrew his allergy, and my middle daughter is now only allergic to two things, but the youngest girl has two allergies: egg and beef, with four food sensitivities.
Next to the doctor’s picture, I filled in their doctor’s office name and wrote the pediatrician’s name underneath. Inside the adjacent box, I wrote the address, phone number, and fax number of the doctors’ office( list underneath each other). I completed the same steps for the dentist, allergy, and childcare box.

Additional Children Information
I did the same for my Lil guy’s information, except some of the doctors are different. On his printable, I added the picture of the ear for his Ear, Nose, and Throat (ENT) doctor and a picture of a vision for his eye doctor.

School Information for Kids
The last printable contact information for the school is in the top box next to the school image.

Lastly, I filled in the information for the before and after school. The yellow box is for the name/s of children that attend that school.
